PLEASE NOTE: We direct bill for most insurance extended health benefits. Please be sure to contact your provider to see what is covered under your extended health benefits.
CANCELLATION POLICY - 24-HOURS NOTICE: is required for cancelling or rescheduling appointments. There is a full charge for late cancellations/no-shows when an appointment is missed or cancelled without 24 hours notice. When booking appointments we require all patients to provide their credit card information. Our staff does not have access to your credit card information and your credit card will not be charged without verbal or written notification.
